I understand that by asking to be involved in the Green Heron Labs (GHL) Mercy Program, I will be contacted by a representative of GHL via email for a discussion of my medical needs. I understand that this discussion will go over relevant and private medical information, and I agree to this conversation.
I understand that there are a lot of people nationwide who are in horrible pain, that GHL can’t help them all, and that might include me. I may have to wait a long time – over a year – and may not be accepted into the program at all. I also understand that the judgement of whether I am accepted into the program is subjective and made by the CEO and that his personal opinion is final, unfair as I may believe it to be.
If accepted into the program, I also agree to inform my GHL representative of how the medicine has helped me, any problems I may be having and disclose dosage information and other details.
I absolve GHL of any problems I may encounter with the medicines they provide me free of cost, and this includes any problems from medicinal interactions as well as addiction problems (psychological or physical).
Lastly, I understand that the Mercy Program can include older, less potent or experimental medicines. It’s what GHL has on-hand that could help me, and I understand it’s the best they have to offer for free. This is a short term program for many different people who desperately need even a day of relief ; this medicine takes 10 months to make and it’s hard to come by, so I might not get all the medicine I need to heal completely.
I also understand that GHL will do their very best to help me, as helping people is why the business was founded in the first place.
By applying I agree to these conditions, and understand there could be additional conditions if I am accepted into the program.
